Case Management is a service for adults, women who are pregnant and children age three (3) and older who have a serious emotional disturbance, mental illness, or a substance abuse related disorder.

Case Management assists recipients in gaining access to necessary care and medical, behavioral, social, housing and other services appropriate to their needs. It is individualized, person-centered, empowering, comprehensive, strengths-based, and outcome-focused.

The functions of Case Management include: Case Management Assessment; Person Centered Planning; Referral/Linkage; and Monitoring/Follow-up.
